Writers research; clinicians verify. A reviewer can hold a piece indefinitely, and regularly does.

We cite current consensus guidance first. Where evidence is thin, we say so plainly rather than filling the gap with confidence.

Every substantive correction is dated and left on the page. Quietly editing a mistake is not a correction.
