Canine diabetes mellitus most commonly results from an absolute deficiency of insulin due to immune-mediated destruction of pancreatic beta cells, meaning affected dogs, unlike many diabetic cats, almost always require insulin injections rather than oral medication or diet alone. Middle-aged, entire female dogs and certain breeds including Samoyeds, Australian Terriers and Miniature Schnauzers are over-represented, and untreated diabetes progresses relentlessly toward the life-threatening complication of diabetic ketoacidosis.
Why Dogs Need Insulin, Unlike Many Cats
Insulin normally allows glucose to enter cells for energy production, and its absence forces the body to break down fat for fuel instead, producing ketone bodies as a by-product. Because canine diabetes almost always stems from beta cell destruction rather than insulin resistance, dietary management alone cannot restore adequate insulin production, and exogenous insulin injections become a permanent, non-negotiable part of treatment from diagnosis onward.
Persistently high blood glucose exceeds the kidney's capacity to reabsorb it, causing glucose to spill into the urine and drag water with it osmotically, which explains the dramatic increase in thirst and urination that is usually the first sign owners notice.
Recognising the Classic Triad of Signs
- Markedly increased thirst and urination
- Weight loss despite a normal or increased appetite
- Cloudy eyes from diabetic cataract formation
- Lethargy and reduced interest in exercise
- Recurrent urinary tract infections from glucose-rich urine
Diabetic cataracts can develop with striking speed, sometimes within weeks of diagnosis, because excess glucose is converted to sorbitol within the lens, drawing in water and clouding it, which is why some dogs present to the vet for sudden blindness rather than the thirst and weight loss that came before it.
Confirming Diagnosis and Ruling Out Ketoacidosis
Diagnosis requires persistent fasting hyperglycaemia alongside glucosuria, since stress alone can transiently elevate blood glucose in dogs, though this stress hyperglycaemia is less pronounced and less persistent than in cats. Fructosamine testing, which reflects average blood glucose over the preceding two to three weeks, helps confirm chronic elevation and later serves as a useful monitoring tool that is less affected by short-term stress than a single glucose reading.
Any newly diagnosed diabetic dog showing lethargy, vomiting or reduced appetite should be assessed promptly for diabetic ketoacidosis, a life-threatening complication involving significant dehydration, electrolyte derangement and blood acidification that requires immediate hospitalisation rather than outpatient management.
Building a Reliable Twice-Daily Insulin Routine
Successful long-term management depends on establishing a consistent daily routine around feeding and insulin administration rather than making frequent ad hoc adjustments.
- Twice-daily insulin injections given roughly twelve hours apart
- Fixed meal times and consistent diet composition around each injection
- A high-fibre, consistent-carbohydrate diet to blunt post-meal glucose spikes
- Regular veterinary blood glucose curves to guide dose adjustments
Home Monitoring Without Constant Vet Visits
Consistency is the single most important factor in stable diabetic control, since varying meal times, portion sizes or exercise intensity from day to day makes it far harder to find and maintain an effective insulin dose. Owners are taught to recognise the signs of hypoglycaemia, including weakness, trembling and disorientation, and to keep a glucose source such as honey on hand for emergency use, since insulin overdose is a real risk during dose-finding periods.
A continuous glucose monitor, applied to the skin and read via smartphone, has become an increasingly popular home tool that reduces the stress of repeated blood sampling while providing detailed data for the vet to fine-tune insulin dosing.
When to seek help urgently
- Vomiting, lethargy or rapid breathing suggesting ketoacidosis
- Weakness, trembling or collapse suggesting hypoglycaemia
- Sudden onset of blindness from rapidly progressing cataracts
- Refusal to eat combined with a scheduled insulin dose already given
Diabetes is very manageable with a consistent routine, but getting the insulin type, dose and timing right requires close collaboration with your vet, particularly in the weeks immediately following diagnosis.
