Canine Epilepsy: Living Well Between Seizures
Idiopathic epilepsy is the most common cause of recurrent seizures in dogs, typically emerging between six months and six years of age in breeds such as Border Collies, Labradors and Beagles, where a genetic basis is suspected but no structural brain lesion can be identified.

Idiopathic epilepsy is the most common cause of recurrent seizures in dogs, typically emerging between six months and six years of age in breeds such as Border Collies, Labradors and Beagles, where a genetic basis is suspected but no structural brain lesion can be identified. Seizures result from a sudden, excessive and synchronised electrical discharge across networks of neurons, and while a diagnosis can feel frightening, most affected dogs live full, active lives between episodes with appropriate management.
What Happens in the Brain During a Seizure
A seizure begins when a group of neurons becomes hyperexcitable and fires excessively, recruiting neighbouring neurons into a synchronised electrical storm that overwhelms normal inhibitory control. In generalised tonic-clonic seizures, this activity spreads across both cerebral hemispheres, producing the whole-body stiffening and paddling movements most owners recognise, along with loss of consciousness.
Idiopathic epilepsy is thought to arise from inherited abnormalities in ion channels or neurotransmitter receptor function that lower the seizure threshold, though the precise genetic mutations have only been identified in a handful of breeds so far.
The Three Phases of a Seizure Event
- A pre-seizure aura phase with restlessness or seeking out the owner
- Rigid limb extension followed by paddling or jerking movements
- Loss of bladder or bowel control during the seizure
- Excessive drooling and jaw champing
- A post-ictal phase of disorientation, temporary blindness or pacing lasting minutes to hours
The post-ictal phase can be more distressing for owners than the seizure itself, since a dog may appear blind, wobbly or anxious for anywhere from a few minutes to over a day afterward, even though full recovery is expected.
Ruling Out Structural and Metabolic Causes
Idiopathic epilepsy remains a diagnosis of exclusion, requiring bloodwork, bile acid testing and often MRI of the brain combined with cerebrospinal fluid analysis to rule out structural causes such as brain tumours, encephalitis or portosystemic shunts, particularly in dogs presenting outside the typical age range. Keeping a detailed seizure diary noting date, duration, and any triggers is invaluable, since it helps distinguish true idiopathic epilepsy from reactive seizures caused by toxin exposure or metabolic disturbance.
Video recordings of episodes on a phone are extremely useful for vets, since seizures rarely occur during a consultation and the specific movement pattern helps differentiate seizures from other events like syncope or vestibular episodes.
Choosing and Adjusting Anticonvulsant Therapy
Treatment aims to reduce seizure frequency and severity to a level compatible with a good quality of life, since a complete cure is not typically achievable.
- Phenobarbital or potassium bromide as first-line long-term anticonvulsants
- Levetiracetam as an add-on for breakthrough or cluster seizures
- Regular blood level monitoring to keep drugs within the therapeutic range
- Rectal or intranasal diazepam prescribed for emergency use at home
Supporting Your Dog Between Episodes
Most dogs starting treatment need six to eight weeks before a therapeutic blood level is reached with phenobarbital, and periodic monitoring of liver enzymes and drug levels is essential since phenobarbital is metabolised by the liver and can cause hepatotoxicity at high doses over time. Treatment decisions typically begin once a dog has more than one seizure every few months, or experiences cluster seizures or status epilepticus, since starting medication earlier in these cases improves long-term control.
A consistent daily routine, adequate sleep and avoidance of known triggers such as flashing lights in photosensitive dogs can help reduce seizure frequency alongside medication, and most well-managed dogs continue to enjoy normal exercise and daily life.
When to seek help urgently
- Any single seizure lasting longer than five minutes
- Cluster seizures, meaning more than one seizure within 24 hours
- Failure to regain normal consciousness between seizures (status epilepticus)
- Seizure accompanied by high fever or bluish gums
Epilepsy is a manageable, lifelong condition for most dogs, and working closely with your vet to find the right medication and monitoring plan gives the best chance of a stable, good-quality life between seizures.
