Mange in dogs is caused by two very different mites that produce superficially similar hair loss and skin irritation but require entirely different management strategies. Demodex canis is a normal resident of hair follicles in almost all dogs and only causes disease when the immune system fails to keep its numbers in check, whereas Sarcoptes scabiei var. canis is a highly contagious, burrowing mite that causes intense itching in any dog it infests, regardless of immune status.

Two Mites, Two Very Different Mechanisms

Demodex mites live within hair follicles and sebaceous glands in virtually all healthy dogs without causing any problem, kept in check by normal immune surveillance. Disease, demodicosis, only develops when local or systemic immune suppression, whether from youth, genetics, or an underlying illness such as hyperadrenocorticism, allows mite numbers to proliferate unchecked within the follicles, causing follicular rupture and secondary bacterial infection.

Sarcoptes mites, by contrast, actively burrow through the epidermis to lay eggs, and their faecal matter and secretions provoke an intense hypersensitivity reaction; this mite is also zoonotic, meaning it can cause a transient itchy rash in humans in close contact with an infested dog.

Localised Versus Generalised Demodicosis

  • Patchy, moth-eaten hair loss around the face and forelimbs (demodectic)
  • Intense, relentless itching disproportionate to visible lesions (sarcoptic)
  • Thickened, crusted skin at the ear margins and elbows (sarcoptic)
  • Secondary pustules and odour from bacterial overgrowth (demodectic)
  • Rapid spread to other household dogs within days (sarcoptic)

The single most useful clue in distinguishing the two at home is itch intensity: localised demodicosis is typically mild and not especially itchy, while sarcoptic mange produces some of the most severe, relentless scratching seen in veterinary dermatology.

Why Sarcoptic Mange Itches So Intensely

Deep skin scrapes examined under the microscope remain the primary diagnostic tool for both conditions, though Demodex mites are usually found easily given their high numbers within follicles, whereas Sarcoptes mites are notoriously difficult to find, with some studies suggesting scrapes detect fewer than half of true infestations. Because of this diagnostic difficulty, vets often treat presumptively for sarcoptic mange based on history and clinical signs alone, especially when index of suspicion is high and scrapes are negative.

Serology testing for Sarcoptes-specific antibodies is available and can support a diagnosis in negative-scrape cases, though cross-reactivity with other mites can occasionally produce false positives.

Skin Scrapes and the Diagnostic Challenge

Because the underlying cause differs so fundamentally, effective treatment requires an accurate diagnosis rather than a generic anti-parasitic approach.

  • Isoxazoline parasiticides (such as afoxolaner or fluralaner) for both mite types
  • Investigation and management of underlying immune suppression in generalised demodicosis
  • Environmental decontamination and treatment of all in-contact dogs for sarcoptic mange
  • Antibiotics or medicated shampoos for secondary bacterial infection

Treatment Pathways That Diverge Sharply

Localised demodicosis in puppies often resolves spontaneously as the immune system matures, whereas generalised demodicosis requires months of isoxazoline treatment alongside identifying and correcting any underlying immunosuppressive condition, since relapse is common if the root cause is not addressed. Sarcoptic mange typically responds quickly to isoxazoline treatment, often within two to four weeks, but because it is so contagious, every dog in contact with the affected animal needs simultaneous treatment even if not yet showing signs.

Adult-onset generalised demodicosis in a dog with no prior history is considered a red flag for underlying disease such as Cushing's syndrome, hypothyroidism or an immunosuppressive cancer, and warrants a broader diagnostic work-up beyond the skin itself.

When to seek help urgently

  • Widespread skin infection with fever or lethargy
  • Rapid, severe self-trauma from uncontrollable itching
  • New, unexplained generalised demodicosis in an adult dog
  • Signs of mite spread to human family members (itchy rash)

Both forms of mange respond well to modern treatment once correctly identified, so a proper skin scrape and clinical assessment from your vet is the fastest route to an accurate diagnosis and relief.
